The range of Remember-I'm-Me Care Charts has been designed to assist in the care of those unable to communicate their own needs.

Already used by over 800 care homes throughout the UK, they have also been adopted by both community hospitals and acute hospitals, to assist in the care of people with dementia, and have received a number of Awards for Innovation.


Origins of Care Charts UK

Founder Zoe Harris developed the first Remember-I'm-Me Care Chart in 2010 to help with her husband's care. She went on to establish Care Charts UK as a social enterprise to develop products designed to improve the quality of life of carers and those they care for.
